LP Partner - Resource Request

This form is used by our partner organizations to request the use of church resources/equipment for an offsite event* (an event not taking place at LifePointe Church).

*PLEASE NOTE: Due to the volume of requests and limited availability of our resources, we are unable to lend resources for personal purposes.
