Facility Use Request // Ministry Partner Event

Fill out this form if you are interested in using our facility for your event.* 

  • Facility requests are approved on a “first come, first served” basis.
  • Submission of this form is not a guarantee of approval for your event. 
  • The Facilities Manager will contact you for approval or denial of your request. Please allow up to 3 weeks response time.
  • If you are approved, additional paperwork may need to be completed to confirm your event.

*NOTE: Due to the volume of requests and limited availability, our facility is only used for such events as memorials and wedding ceremonies. It is not available for personal (non-ministry) use – this includes baby showers, wedding showers, birthday parties, grad parties, etc.

Include number of people anticipated, number of rooms, and any resources or equipment you may need (i.e. tables & chairs, A/V equipment, etc.)

By checking the box below, I understand this request is PENDING and is not a guarantee that I am approved to use the facility for my event. I will be contacted by the Facilities Manager for final approval/denial.
